1. COVERED DIAGNOSIS

• Cancer (adult or child)

2. WHAT WE FUND

• Medical Expenses (defined)
• Transportation (travel expense for treatment)
• Caregiver Expenses
• Living Expenses
• Nutritional Expenses

3. APPLICANT ELIGIBILITY REQUIREMENTS

• Must live in Hampton Roads
• Must provide individual or family adjusted gross income.
• Primary Care Doctor/Patient will have to sign and attest to the diagnosis.
• Currently undergoing a treatment plan under the prescribed plan of care or about to start.
• Applicants must be 18 years old, or a legal guardian can submit the application on behalf of minors.

4. AWARD DETAILS

• Maximum award amount: $1000.00 per year
• Applicants can apply once every 6 months.
• Applicants will be contacted within 10 business days regarding the decision.
